New or Replacement Process
If you are new to PAP or an established user, we will help you get started with us.
If you need to renew your CPAP and supply prescription, or if you are wanting to establish care with us, we will need your contact information in addition to the items outlined below. In all cases, we will need a complete prescription, which includes the pressure of the machine and supplies listed out individually.
If you are new to CPAP and just getting started, or you want to switch to us for CPAP supplier, we will need the following:
- Copy of your sleep study
- Copy of the notes from your visit with your referring provider (should be within the last six months if this is for a new CPAP machine and not supplies only)
- A complete prescription (should be within the last six months if this is for a new CPAP machine and not supplies only)

If you are replacing your sleep equipment
To be eligible to receive a new CPAP machine, your current machine needs to:
- Be older than 5 years.
- Not work properly.
- Be evaluated by a CPAP clinician to get information from the machine.
- Not be repairable.
We will need to have a new prescription if you want to get a new CPAP machine.
